Bowling Green Public Library provides several forms and request options for patrons and community members.

Forms may be completed in person at the library or downloaded when available.

Event Photography Notice

Library programs and public events may occasionally be photographed or recorded for promotional, educational, or archival purposes.

By attending public library events, participants acknowledge that photographs or video may be taken and used by Bowling Green Public Library in print, online, or on social media.

If you prefer not to be photographed, please notify library staff at the beginning of the event.
